REMINDER: You must call the Benefits Office at 549-4900, ext. 248 within 30 days of these changes:

  • Marriage or divorce
  • Birth or adoption of a dependent
  • Dependent is no longer enrolled as a full time student
  • Dependent reaches his/her 23rd/25th birthday as a full-time student. (See specific plan documents regarding age.)
  • Dependent student graduates

DENTAL INSURANCE

This program is available to salaried employees working at least 75% or more of full time and covers most dental costs including orthodontia.

Dental Claims Administrator Website

The program is administered by J.J. Stanis, a contracted third party administrative company, who processes all claims. For the fastest service, use this website or call 1-877-470-3715. At the website, you can:

  • Find list of Participating Providers:
    • Go to Dental Networks, then
    • Click on Stanis Net Plus from dropdown
    • Enter information in Directory Search
  • Learn Status of Dental Claims:
    • Go to Services, then
    • Click on Claims Access from dropdown
    • click on Suffolk BOCES
    • First time User Name is member's Social Security number and Password is full birth date (xx/xx/xxxx). After you log in the first time, you will have the option to change your information.

Dental Benefits Listed

Open for a list of dental benefits based on your bargaining unit. Includes orthodontic, calendar year maximum, deductible, % of reasonable and customary rates.
Dental/Major Medical - Dependent Student Status Form

To maintain coverage for dependent students, proof of student status is required. Unless the appropriate form is filed, the carriers will not process claims. Employees must complete this “STUDENT STATUS FORM” for:

  • Full-time students who are 19 to 25 years of age and who are dependents of teachers, administrators and supervisors
  • Full-time students who are 19 to 23 years of age and who are dependents of all other personnel
  • Must be completed for BOTH the fall and spring semesters for EACH year. Note that Part A must be completed by the employee; Part B must be completed, including the official seal, by the accredited academic institution.


EXCESS MAJOR MEDICAL AND VISION CARE

Benefits vary depending on the employee's bargaining unit.

FLEXIBLE BENEFITS

This program is available to salaried employees and permits you to apply part of your salary to the payments you may make for uninsured medical expenses and dependent care expenses in a before tax basis.

The Plan also allows you to have your payroll deductions for items such as health insurance treated as before tax payments.

Claim Form and Instructions Sample only; obtain NCR form from supervisor. Complete for reimbursement for medical expenses that have not been reimbursed or are not reimbursable under other coverage. Also use this form for reimbursement for dependent care expenses that are coverable under IRS code. Send to Benefits Office. Detailed instructions are on page 2.
Annual Election Form Download this form. To participate in the Flexible Benefit program, complete, sign and return form to the Payroll Office by the deadline date each November/December.


HEALTH INSURANCE

BOCES provides health insurance for salaried employees and some casual employees with the cost being shared pursuant to the various unit contracts. Employees may also qualify to continue their health insurance after retirement if they meet curtain conditions for service with Western Suffolk BOCES.

There are two options for health insurance:

  • NYS Empire Plan. This is part of the NYS Health Insurance Program for NY State and Local Governments.  The plan is administered by the NYS Dept. of Civil Service through contracts with service providers.
  • HIP

LIFE INSURANCE

Western Suffolk BOCES provides a group term life and accidental death and dismemberment insurance program through its Third Party Administrator (J.J. Stanis & Co.) and Standard Life Insurance Co. The amounts of coverage are determined by the bargaining agreements with various employee groups and are fully paid for by Western Suffolk BOCES. There is also an optional contributory plan in which the employee can enroll for additional insurance at his/her cost. Read the summary and policy for details.

LONG TERM DISABILITY

This program is available to salaried employees working at least 75% or more of full time. For employees who are disabled beyond a minimum period of time and who have exhausted all available sick leave, this program provides a cash benefit based on the employee's salary. To claim payment under this benefit, a disabled employee will need to document his/her disability, including at BOCES option, a review by an independent medical examination.

OTHER BENEFITS--AFLAC

AFLAC offers a number of optional benefit programs that are available to those in bargaining units 1, 2, 4, 5, 6 (Unrepresented) and 7. AFLAC benefits are paid for completely by the employee through payroll deductions.

When can I enroll?

New Hires:

Enrollment in AFLAC benefits is available to newly hired employees in the above units for 90 days following their date of hire.

Current Employees:

Enrollment in AFLAC benefits is available to current employees each year during the open enrollment period of November and December for a January 1 start.
